If you would like to file a consumer complaint against a business in the state of Texas, please contact the Texas Attorney General's Office. You can file a complaint online or learn more about your consumer rights.
Your Rights
You have the right to request a copy of your mental health records. Records will be provided in accordance with Texas law and applicable privacy regulations.
Requests may be submitted via mail, in writing, or through the messaging feature of your secure client portal.
Important Notice
All requests must unambiguously state that the purpose is to request delivery of your mental health records. Sign your ROI first.
A signed Release of Information (ROI) is required before records are provided.
Records Policy Details
A fee of $2.00 per page of client records will be assessed to cover copy and printing charges.
Please submit your records request with plenty of advance notice. Allow 2 weeks for delivery after your request has been processed.
Requests must clearly state their purpose and must not mix other questions or unrelated matters. One purpose per request, please.
Under certain limited circumstances listed in 45 CFR 164.524(a)(2)–(a)(3), access to all or a portion of the requested PHI may be denied.
